Parents do not search procedure names first. They search a white pupil, a cloudy little cornea, or “our baby was in NICU — does the retina need a check?” Those three organic paths are congenital cataract, congenital glaucoma, and retinopathy of prematurity (ROP). They share one rule: time is the resource.

Congenital cataract: when the red reflex is wrong

A cataract in a baby or toddler is a lens that should be clear and is not. A white pupil (leukocoria), a missing or dull red reflex in photos, or a wandering eye can be the tell. Adult cataract blogs do not apply — the visual system is still wiring. Delay is how amblyopia becomes the second disease.

Congenital glaucoma: cloudy cornea, tearing, light-fear

Infant glaucoma is not the slow adult nerve story on glaucoma surgery. Pressure can enlarge and cloud the cornea. Parents notice tearing, photophobia, a bigger-looking eye, or a dull cornea.

Goniotomy or trabeculotomy — operations on the infant drain — are specialist pediatric glaucoma tools. Drops may bridge. They are not the whole plan.

A 4-month-old with “always watery eyes” needs this diagnosis ruled out, not only a blocked-duct massage video from a family group. Adult watering that is a drain block sits on DCR.

ROP: the NICU graduate’s retina

Retinopathy of prematurity is abnormal vessel growth in a premature retina. Screening schedules exist for a reason. Laser, injections, or vitrectomy in severe disease are retina-pediatric tools, often while the baby is still in or just after NICU.

If a discharge summary skipped the ROP check, that is not a paperwork footnote. Call.

If a screen was missed, call. A late screen is still a screen.

Adult diabetic retina and detachment pages are a different age group: diabetic retinopathy, flashes and curtain.

What a pediatric evaluation should feel like

Time. Drops. A look at the red reflex, cornea, pressure when possible, and the retina. Honesty that some exams in very young children need a specialist setting.
