Squint Surgery: When a Turning Eye Needs More Than “They Will Grow Out of It”

A turning eye is a search parents actually make. Relatives still answer with “wait”. Strabismus (squint) is a misalignment of the two eyes. In a child, the brain may switch one eye off. In an adult, new double vision is a different, sometimes urgent, clock. Squint surgery is one tool after measurements — not a default, not a cosmetic afterthought only.

What squint is doing to a child’s vision
When one eye turns in, out, up or down, the brain receives two pictures that will not fuse. Children often suppress one image. That suppression can become amblyopia (lazy eye): even with a straight-looking eye later, the parked eye never learned. Time is the resource.
A 4-year-old with an intermittent inward turn still needs a cycloplegic refraction — drops that reveal the true power — not a six-month family debate.
The usual medical order:
- Glasses (especially when high hyperopia drives an inward turn)
- Patching or atropine for amblyopia
- Then, in selected cases, eye-muscle surgery

Adult squint and new double vision
Long-standing adult squint (cosmetic alignment, or decompensated childhood tropia) is one story. Sudden double vision is another: nerve palsy, thyroid eye disease, myasthenia, trauma. Do not park a new vertical double behind “I need a stronger pair”. That is not LASIK territory.
A 36-year-old consultant we saw had diplopia only when tired on a long commute. Intermittent does not mean imaginary. Prism in glasses can be a trial. Surgery is a later, measured decision.
If the eyes look pushed forward, or thyroid is already in the file, read thyroid eye disease and orbit as well.
What a proper alignment evaluation includes
- Vision in each eye
- Cycloplegic refraction in children
- Cover tests at near and distance
- A look at the retina and optic nerve so the turn is not hiding something worse
- In adults, a cranial-nerve and medical history
You should leave knowing: glasses first vs patching vs operate vs investigate the cause.
Measurement ke bina theatre nahi — no millimetres guessed from a phone video, though a video of the turn at home is useful extra data.
What eye-muscle surgery does and does not do
The surgeon weakens or tightens extraocular muscles to rotate the eye. Alignment can improve. Stereopsis (3D) may or may not return in older children and adults. A second procedure is sometimes needed as the child grows. Over- and under-correction exist.

Myths
“They will grow out of a true squint.” Some intermittent patterns change. Many do not, and waiting can cost binocular vision.
