What insurers actually look at
Function: the lens is cloudy enough to need replacing so you can see, work, or not fall. Appearance language in the notes — “elective premium package,” “patient desired better appearance” — is how files get rejected even when the cataract is real.

Not every cloudy complaint is a cataract. A scarred cornea, an uncorrected glasses number, or a retina problem can sit in the same history. That is why evaluation comes before a TPA file.
