What you are actually paying for
An intravitreal injection is a sterile procedure: anaesthetic, antiseptic, a measured dose into the vitreous. The drug is most of the money. The visit is usually day care in Indian private hospitals — you go home the same day.

Why two quotes for the “same” injection
The lower end of a steroid band is often a simpler drug in a straightforward day-care slot. The upper end is a busier OT, a repeat, or a case that needed extra imaging the same morning.
Ozurdex costs more because you are paying for an implant, not only a syringe. Insurers sometimes apply a sub-limit to implants. That is why a family hears “covered” and still pays a gap. OCT, a consult, and drops billed the same morning are often OPD even when the injection is day care. That split is the usual surprise. It is also why “injection package ₹X all-in” from a banner is not a number we will print.
Will insurance take the injection?
Usually yes when it is listed day care and the notes show medical necessity (oedema, uveitis grade, endophthalmitis). Usually not for the outpatient tests around it, unless you have a rider. When an injection is the wrong next step
Sudden vision drop, a shower of floaters, a curtain: that may be a retinal detachment, not a booking for a steroid shot. Pain, pus, or a white layer in the front of the eye after a recent injection or cataract is infection until proven otherwise — same-day, not a cost page.
